Context: The Unified Health System (SUS) is a free-of-charge, public policy that found in Harm Reduction (HR) a way to comprise embrace the population of drug users who did not adhere engaged into abstinence treatments and remained outside the system.
HR was anchored: 1. in Public Health in the 1970s, including social determinants in the health-disease process; 2. in the Psychiatric Reform ofin the 90s, with deinstitutionalization and the whole full exercise of civil rights, and 3. in the Alcohol and Drugs Policy in 2003, with the expansion of services in a Psychosocial Care Network (RAPS), 4. in Anti-prohibitionist legal currents. In political tensions over care models, those who are most penalized by mass incarceration due to drugs are the poor and black population. In addition to the problems of discontinuity of financing.
Model Description: HR integrates knowledge from harm reducers, users, and other professionals to plan shared care. The HR focus is on the user and in their network of relationships, on the care taking HR respects the users subjective time, their needs and desires. The use of the fField dDiary is taken used forto supervision, to team meetings, and to the intersectoral network to discuss case by case. It proposes a cultural debate on the social imaginary of users and drugs.
Evidence: This model expands access and adherence engagement into the services, social rights and new therapeutic possibilities, HIV and Hepatitis epidemiological control, and social vulnerability reduction of social vulnerability. It encourages users; self-organization and inclusion in Rights Councils, in the effective formulation of drug policies.
Conclusions: Disseminating HR RD practices and productions is necessary to strengthen it as a work guideline. It is still necessary required to transform HR into State Policy to also insert it in contexts of violence, in justice programs and in lifestyle and consumption.
